No. The PDF is opened by pdf.js, the same PDF engine Firefox uses, running inside your tab. There is no upload step, and closing the tab discards everything.
Because it was scanned or photographed. A scanner makes a picture of each page, and a picture has no words in it until OCR (optical character recognition) reads them. The tool checks every page and names the ones with no text layer, so you know which pages need OCR.
A PDF breaks lines where the page ran out of width. Pasted into a document, those breaks stay and the text no longer reflows. Join into paragraphs keeps the breaks between paragraphs, found from the larger gaps between lines, and joins the lines inside each one. You can switch between the two after the text is out, without running it again.
Text comes out in the order the PDF stores it, which for most documents is reading order. Two columns usually come out one after the other. A table comes out row by row, with the cells of a row on one line. The layout itself, borders and alignment, is not text, so it does not come across.
